Complete the following forms and return them to the Head Coach:

• Physical Therapy Consent to Treat

• Emergency Contact Form

• Consent to Treat Form

All necessary forms can be found on the left side of the page.

Athletes are required to take a swim test before they can go out on the water with the team; typically, this is done early in the second week of school.
